The Hope Foundation’s Board of Directors regularly requests input from the SWOG community in order to address unmet group needs.

Please complete the form below to submit your project concepts to SWOG leadership and the Foundation Board for the 2019 funding year (January-December).

Initial eligibility requirements are included below. We will reach out for more detail should the project be appropriate for the current funding cycle.
